Due to high demand and wait times for the ASD Diagnosis service, we open up availability on a periodic  basis. We manage this process through a waitlist which serves as an initial intake process so that we can better understand your needs and arrange for a suitable team of practitioners (subject to their availability). 

STEP 1: REFERRAL LETTER FROM MEDICAL PRACTITIONER 

You must obtain a referral letter from the client's medical practitioner.

This medical practitioner could be a General Practitioner or Paediatrician or other primary doctor who is currently treating the client and understands the clients circumstances well. 

This referral letter from the client's medical practitioner must contain specific information as follows.

• Express referral of the client for consideration for an ASD Diagnosis; and

• Comments from the medical practitioner in respect of the physical examination and other medical tests undertaken on the client to make it clear as to whether there are medical causes for the behaviours or characteristics that prompted the referral; and

• Comments from the medical practitioner outlining the need for the timely assessment for the client.

STEP 2: APPLY FOR THE ASD DIAGNOSIS PACKAGE WAITLIST

Using the form below, please submit all the information requested in respect of the client.

STEP 3: OBTAIN CONFIRMATION OR A NOTIFICATION REGARDING AVAILABILITY

Once the form is submitted, our team will contact you confirm whether you are on the waitlist for the next round of ASD Diagnostic Packages or to advise if there is limited availability of appropriate practitioners to meet the client's needs. Please do not assume that you have been accepted to the waitlist unless our team has confirmed the acceptance with you.

Please note, the standard ASD Diagnosis Package does not include undertaking the Wechsler Intelligence Scale for Children (WISC-V). If a specific intellectual ability test is required, this will need to be booked additionally and separately.
